Invoice templates, duplicates & batch PDFs

Speeding up repeat billing.

Templates: in the editor, Save as template stores the line items, markups and terms; apply from the template menu when creating new invoices.
Duplicate: copies an invoice as a new draft (contract visit links are not carried, so you can't double-bill visits by accident).
Batch PDFs: select several invoices on the list to download them as one combined PDF for month-end.
